Job Summary
A healthcare company is in need of a Remote Personal Health Nurse Case Manager in Frisco.
Must be able to:
- Determine and document member’s goals
- Engage the member in the personal health management process
- Develop an ongoing individualized plan of care
Must meet the following requirements for consideration:
- Ability to travel 5-30% of the time, depending on the client assigned
- RN (Registered Nurse) license
- Current active professional license in the state of residence
- Eligibility for unrestricted professional licenses in all states
- Bachelor degree in a health related field
- Three to five (3-5) years of diverse clinical experience
