A non-profit health care company has a current position open for a Remote Population Health Pharmacy Coordinator in Arlington Heights.
Core Responsibilities of this position include:
- Coordinating clinical workflows to meet patient care needs
- Designing, planning, educating and implementing safety and performance improvement initiatives
- Providing clinical support to others for more complex cases
Qualifications for this position include:
- Bachelor's Degree in Pharmacy
- 5 years of experience in pharmacy and chronic disease management/managed care including completion of post PharmD residency program
- Pharmacist license issued by the state in which the team member practices
- Strong computer skills
- Excellent presentation, facilitation, and training development skills
