Job Summary
A population health management solutions company has a current position open for a Remote Population Health Specialist.
Candidates will be responsible for the following:
- Providing patient health promotion, and coordinate care
- Supporting Population Health Manager in successful implementation of provider engagement initiatives or programs
- Actively participating in workflows to support gap closure and other targeted provider engagement initiatives
Skills and Requirements Include:
- Minimum of 1 year of healthcare experience
- Bachelor's degree in Healthcare Administration, Business Administration or related field or equivalent experience
- Professional and effective communication skills with multi-disciplinary teams and patients
- Ability to operate in a fast-paced and dynamic environment
- Knowledge of the U.S. healthcare system
- Strong organization skills, detail-oriented, and ability to consistently meet and track timelines and objectives
