Job Summary
A provider of home health benefits management services needs applicants for an opening for a Remote Post Acute Care Nurse Coach.
Individual must be able to fulfill the following responsibilities:
- Provide education, coaching and care coordination
- Manage an assigned case load of Post-Acute Care patients
- Administer initial assessments, progress surveys and discharge surveys
Position Requirements Include:
- Expertise in Telephonic Case Management and knowledge of URAC standards
- Registered Nurse with licensure in the state(s) of practice
- Broad knowledge of health care delivery/managed care regulations
- Prior Case Management/review experience
