Job Summary

A health insurance company is seeking a Remote Post Acute RN Case Manager.

Must be able to:

  • Develop appropriate Discharge Plan
  • Apply appropriate clinical criteria in making determinations of level of payment
  • Conduct assessment to determine what needs or barriers customer may have at discharge

Qualifications Include:

  • Must hold an active, unrestricted RN license in the state of Tennessee
  • Must have at least 3 years of clinical experience
  • Must be able work autonomously
