Job Summary
A company is looking for a Remote Primary Care Liaison.
Key Responsibilities:
- Act as the primary point of contact and care advocate for patients, families, facility partners, and providers
- Proactively manage patient care coordination, including appointment scheduling and follow-up care
- Maintain comprehensive and accurate patient documentation within the Electronic Medical Record system
Required Qualifications:
- Bachelor's degree required, preferably in Healthcare Administration, Health Sciences, or Public Health
- 2+ years of healthcare coordination, patient services, or clinical operations experience required
- Demonstrated understanding of primary care workflows and medical terminology
- Proven ability to manage competing priorities in a fast-paced environment
- High proficiency with healthcare technology platforms, including EMR systems
