Job Summary

A company is looking for a Remote Primary Care Liaison.

Key Responsibilities:
  • Act as the primary point of contact and care advocate for patients, families, facility partners, and providers
  • Proactively manage patient care coordination, including appointment scheduling and follow-up care
  • Maintain comprehensive and accurate patient documentation within the Electronic Medical Record system
Required Qualifications:
  • Bachelor's degree required, preferably in Healthcare Administration, Health Sciences, or Public Health
  • 2+ years of healthcare coordination, patient services, or clinical operations experience required
  • Demonstrated understanding of primary care workflows and medical terminology
  • Proven ability to manage competing priorities in a fast-paced environment
  • High proficiency with healthcare technology platforms, including EMR systems
