Job Summary
A company is looking for a Remote Primary Care Liaison.
Key Responsibilities:
- Act as the primary point of contact and care advocate for patients, families, facility partners, and providers
- Manage patient care coordination, including appointment scheduling and follow-up care
- Maintain accurate patient documentation within the Electronic Medical Record system
Required Qualifications:
- Bachelor's degree in Healthcare Administration, Health Sciences, Public Health, or related field preferred
- 2+ years of healthcare coordination, patient services, or clinical operations experience required
- Understanding of primary care workflows and medical terminology
- Proven ability to manage competing priorities in a fast-paced environment
- Strong clinical judgment and ability to triage patient needs
