Job Summary

A company is looking for a Telehealth Physician (Full-Time | M-F | No On-Call).

Key Responsibilities

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
• Create and manage personalized care plans for ongoing conditions
• Collaborate with the integrated care team and ensure thorough documentation in the EHR

Required Qualifications, Training, and Education

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license; multi-state licensure is a plus
• Credentialed with major plans: UHC, Aetna, Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
