Job Summary

A health insurance company is in need of a Remote Provider Group Telephonic Nurse Care Manager 2.

Core Responsibilities of this position include:

• Managing a member's physical, environmental and psycho-social health issues
• Identifying and resolving barriers that hinder effective care
• Ensuring patient is progressing towards desired outcomes by continuously monitoring patient care

Skills and Requirements Include:

• Licensed Registered Nurse (RN) in the (appropriate state) with no disciplinary action
• Prior clinical experience preferably in an acute care, skilled, rehabilitation or provider clinical setting
• Strong attention to detail, excellent time management and organizational skills
• Excellent communication skills both verbal and written
• Proficient with Microsoft Word, Outlook and Excel
• Ability to work independently under general instructions and with a team
