Job Summary

A health insurance company is in need of a Remote Registered Nurse Care Manager.

Candidates will be responsible for the following:

• Managing a census of patients by helping to identify and carry out a plan of care
• Participating in Interdisciplinary Team Meetings
• Conducting telephonic assessments of acute and chronic conditions

Required Skills:

• RN candidates licensed in either the states of Arizona, California, Nevada or Texas
• Must have a minimum 3 years of clinical experience
• Prior healthcare experience serving chronically ill patients
• Compassionate and professional demeanor
• Strong Word, Excel, and database management skills
• Experience working in a fast-growing, rapidly changing environment
