Job Summary

A non-profit healthcare services company is filling a position for a Remote Registered Nurse Care Manager in Henrietta.

Must be able to:

  • Perform comprehensive health assessments/reassessments to identify health and environmental needs
  • Revises care plan as needed based on changes to the members’ physical, mental, or social wellbeing
  • Collaborate with members and develop their individualized short- and long-term goals

Required Skills:

  • Valid NYS driver’s license and ability to drive in all types of weather
  • License and current registration as a Registered Professional Nurse in New York State
  • Long term care and/or managed care experience
