Job Summary

A non-profit healthcare services company is in need of a Remote Registered Nurse Care Manager in Syracuse.

Core Responsibilities of this position include:

• Conducting home/facility visits for comprehensive health assessments/reassessments
• Revising and amending care plan as needed
• Collaborating with members, develops their individualized short- and long-term goals

Position Requirements Include:

• Must travel to homes and facilities as needed to complete duties
• License and current registration as a Registered Professional Nurse in New York State
• Long term care and/or managed care experience
• Ability to work independently, multiple assignments, prioritize workload
• Strong communication skills person to person and by phone
