Job Summary
An independent, not-for-profit health benefits organization has an open position for a Remote Case Management and Population Health Manager.
Candidates will be responsible for the following:
- Developing and implementing clinical programs to achieve improved rates
- Providing clinical expertise in the development of programs and educational material pertaining to clinical management
- Participating in the development of effective strategies, policies, and procedures related to clinical management programs
Skills and Requirements Include:
- 3 years - Experience in Health Care Services specific to UM/CM required
- 3 years - Experience in a leadership/management role required
- 5 years - Health care experience
- Certified Case Manager (CCM ) required
- Proficient in Microsoft Office (Outlook, Word, Excel and Powerpoint)
- Registered Nurse with active license in the state of Tennessee or hold a license in the state of their residence
