Remote Registered Nurse Case Manager

Job Summary

Seeking dedicated Registered Nurses for a full-time, fully remote Case Manager position, where they will manage a caseload of patients with complex conditions, serving as clinical advocates and educators while coordinating care and developing individualized management plans.

Key responsibilities

• Conduct comprehensive telephonic assessments and develop goal-oriented Case Management plans
• Coordinate care across various providers and perform utilization reviews to ensure effective treatment
• Educate patients on their conditions and connect them with community resources to improve health outcomes

Required qualifications

• Active, unrestricted RN compact license and certification as a Case Manager
• Minimum of three years of direct clinical patient care experience
• Proficiency in Microsoft Outlook and basic computer skills
• Reliable high-speed internet connection (minimum 25 Mbps download/upload)
