Remote Registered Nurse Case Manager

Job Summary

Seeking dedicated Registered Nurses for a part-time (20-25 hours/week), fully remote opportunity as a Remote Registered Nurse Case Manager, responsible for managing a caseload of complex and high-risk patients through comprehensive assessments, care coordination, and patient education.

Key responsibilities

• Conduct in-depth telephonic assessments and develop individualized Case Management plans
• Coordinate care across providers and perform utilization reviews to ensure effective treatment
• Educate patients on their conditions and connect them to community resources for support

Required qualifications

• Active, unrestricted RN compact license and certification as a Case Manager
• Minimum of three years of direct clinical patient care experience
• Proficiency in Microsoft Outlook and basic computer skills
• Reliable high-speed internet (minimum 25 Mbps download/upload)
