Job Summary

A healthcare company is searching for a person to fill their position for a Remote Registered Nurse Case Manager .

Candidates will be responsible for the following:

  • Conducting an evaluation of member's needs and benefit plan eligibility
  • Facilitating integrative functions as well as smooth transition to health programs and plans
  • Addressing complex health and social indicators which impact care planning

Applicants must meet the following qualifications:

  • 0-10% travel
  • 5+ years clinical practice experience required
  • RN with current unrestricted state licensure required
