Job Summary
A healthcare company is filling a position for a Remote Registered Nurse Case Manager.
Individual must be able to fulfill the following responsibilities:
- Using clinical tools and information/data review to conduct evaluation of member's needs
- Applying clinical judgement to incorporate strategies designed to reduce risk factors and barriers
- Conducting assessments that consider information from various sources
Must meet the following requirements for consideration:
- Ability to travel within a designated geographic area for in-person case management activities
- Registered Nurse with active state license in good standing within the region
- Minimum 3-5 years clinical practical experience
- Minimum 2-3 years CM, discharge planning and/or home health care coordination experience
- Effective computer skills including navigating multiple systems and keyboarding
- Proficiency with standard corporate software applications, including MS Office applications
