Job Summary
A hospital management company is searching for a person to fill their position for a Remote Registered Nurse Case Manager.
Core Responsibilities Include:
- Creating care plans that include objectives, goals and interventions designed to meet the member's needs
- Assessing the physical, functional, social, psychological, environmental, learning, and financial needs of members
- Evaluating member's progress toward goal achievement
Position Requirements Include:
- Active, unrestricted license as a Registered Nurse in the state in which the CM practices
- BS/MS in Public Health or related field
- 3 yrs minimum Case Management experience in a managed care environment required
- 3 yrs minimum clinical medical/surgical nursing practice within a hospital setting
- Certified Case Manager (CCM) or Certified Professional in Healthcare Quality (CPHQ), or ability to obtain
- Recent (within past 3 years) working knowledge of Milliman Care Guidelines required
