Job Summary
A healthcare staffing and recruiting firm has a current position open for a Remote Registered Nurse Case Manager.
Candidates will be responsible for the following:
- Coordinating and monitoring the care of Health Plan members and develop a nursing plan of care
- Participating in interdisciplinary conferences to review clinical assessments
- Updating care plans and determining follow-up frequency
Skills and Requirements Include:
- Valid, unrestricted state Registered Nurse (R.N.) license
- 2 years' experience with Care Coordination, Case Management and/or discharge planning
- Around 5 years' acute care clinical nursing experience
- Basic computer literacy (MS Office) and typing skills are necessary
- Case Management experience can be from a Health Plan, TPA, Medical Group/IPA, or Commercial setting
