Job Summary
A health management organization is searching for a person to fill their position for a Remote Registered Nurse Health Coach.
Candidates will be responsible for the following:
- Performing the assessment and evaluation of patients with chronic disease by collecting medical and social information
- Making referrals for statewide community resources
- Utilizing motivational interviewing techniques; teaching self-management skills and supporting patients and families
Must meet the following requirements for consideration:
- This position is remote, with travel required to clinics or hospitals as needed
- Two or four-year degree in health care or related field and/or equivalent training and/or experience
- Current Oklahoma RN license without restrictions
