Job Summary

A non-profit health insurance company has a current position open for a Remote Registered Nurse Patient Care Coordinator.

Core Responsibilities Include:

• Coordinating care and facilitating discharge to an appropriate level of care
• Identifying and referring quality issues to Quality Improvement
• Identifying and referring appropriate members for Care Management

Position Requirements Include:

• Current, unrestricted Registered Nurse (RN) Licensure in state(s) of practice
• Completion of an accredited registered nursing (RN) degree program
• Minimum of two (2) years clinical experience
• Basic data entry skills and internet utilization skills
• Working knowledge of Microsoft Word, Excel and Access
• Familiarity of the healthcare field
