Core Responsibilities of this position include:
- Presenting complex patient members for review by the interdisciplinary team
- Contacting members with gaps in preventive health care services and assisting them
- Reviewing member's current medication profile and identifying issues related to medication adherence
Position Requirements Include:
- Registered Nurse (RN)
- 2 years of experience in a clinical setting and case management nursing
- Ability to effectively interact with physicians and other health care professionals
