Job Summary
A healthcare agency is in need of a Remote Resource Coordinator and Supports Planner in Montgomery County.
Must be able to:
- Evaluate client's capacities to assist in the development of plans of service
- Promote positive health behaviors to support optimal health and well-being
- Gather information to assist in the development of plans of service provided to clients
Applicants must meet the following qualifications:
- Performing on-site visits, information gathering, monitoring of plans of service
- Bachelor's degree from a four-year college or university in health/human services or related field
- Must possess oral and written communication skills
- Ability to conduct research on a computer and other sources
- Ability to use Microsoft Office, Excel, and PowerPoint
- Ability to write routine reports and correspondence
