Remote RN Care Manager

Job Summary

Managing an assigned patient panel, the full-time Remote RN Care Manager will provide comprehensive care management, establish empathetic relationships with patients, and advocate for their needs while working remotely in the Mountain Time Zone.

Key responsibilities

• Oversee care management for a patient panel, utilizing nursing expertise and value-based care operations
• Build trusting relationships with patients and families to offer clinical and emotional support throughout their care journey
• Coordinate interdisciplinary care plans and monitor the effectiveness of care management strategies to improve patient outcomes

Required qualifications

• Associate degree in nursing
• Current RN License required; Compact License preferred
• Experience in care management is required
• Certified Case Manager preferred
• Intermediate skills with MS Office Suite, including Outlook and Teams
