Job Summary
To support a growing healthcare team, the full-time Remote RN Care Manager will manage an assigned patient panel, focusing on individual healthcare needs, collaborating with interdisciplinary teams, and advocating for patients while working remotely in the Central and Eastern Time Zone.
Key responsibilities
- Managing the overall care of a patient panel by leveraging nursing expertise and value-based care operations
- Establishing trusting relationships with patients and families to provide clinical and emotional support throughout their care journey
- Coordinating interdisciplinary care plans and monitoring the effectiveness of care management strategies to improve patient outcomes
Required qualifications
- Associate degree in nursing
- Current RN License required; Compact License preferred
- Care management experience required
- Certified Case Manager preferred
- Intermediate skills with MS Office Suite, including Outlook and Teams
