Remote RN Care Navigator

Job Summary

A company is looking for a 100% Remote RN Care Navigator (EST & CST).

Key Responsibilities

• Manage case management specific to kidney health, including member data entry and care plan maintenance
• Educate members on kidney health and related conditions, optimizing renal replacement therapy
• Coordinate care among patients, their support networks, and healthcare providers to ensure positive outcomes

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
