Remote RN Case Manager

Job Summary

A company is looking for a Telephonic Registered Nurse Case Manager - Remote.

Key Responsibilities

• Make outbound and inbound calls to assess members' health status
• Identify gaps in treatment plans and provide patient education for self-management
• Coordinate care and document findings for members

Required Qualifications

• Active, unrestricted RN license in the state of residence
• Willingness to obtain multiple state and/or compact licensure within 60 days of hire
• 3+ years of RN experience in a hospital or acute care setting, or as a telephonic Case Manager
• Computer proficiency, including data entry skills and navigating a Windows environment
• Designated quiet work space and access to secure high-speed internet
