Job Summary

A healthcare provider has an open position for a Remote RN Case Manager in Whatcom Counties.

Core Responsibilities of this position include:

• Completing comprehensive assessments of members per regulated timelines
• Developing and implementing a case management plan in collaboration with the member
• Conducting face-to-face or home visits as required

Required Skills:

• 25- 40% local travel required
• Graduate from an Accredited School of Nursing
• 1-3 years in case management, disease management, managed care or medical or behavioral health settings
• Active, unrestricted State Registered Nursing (RN) license in good standing in the State of Washington
• Must have valid driver's license with good driving record
• All other requirements necessary for this position
