Job Summary
A staffing agency has a current position open for a Remote RN Discharge Planner in Milwaukee.
Core Responsibilities Include:
- Reviewing and monitoring daily inpatient stays per inpatient work processes
- Monitoring inpatient activity daily for outcomes
- Coordinating services, communicating information, and interfacing with health care providers
Position Requirements Include:
- Must be able to travel to Milwaukee offices on occasion
- Current, valid, and unrestricted state Registered Nurse license
- 2 years acute care clinical nursing experience
- Must have 2+ years of recent experience in Discharge Planning or Care Coordination
- Knowledge of guidelines for Medicaid/Medicare and related state programs
- Computer skills to include Microsoft Word, Excel, database use, and basic data entry
