Job Summary
A healthcare company is filling a position for a Remote RN or Licensed Social Worker Case Manager in Cincinnati.
Must be able to:
- Maintain ongoing member case load for regular outreach and management
- Assess for barriers to care, provide care coordination and assistance to member to address concerns
- Conduct telephonic, face-to-face or home visits as required
Applicants must meet the following qualifications:
- 25- 40% local travel may be required
- Graduate from an Accredited School of Nursing
- Bachelor's or Master's Degree
- 1-3 years in case management, disease management, managed care or medical or behavioral health settings
- Active, unrestricted Ohio Registered Nursing (RN) license in good standing
- Must have valid driver's license with good driving record
