Job Summary

A healthcare provider is searching for a person to fill their position for a Remote RN or LPN Care Coordinator.

Core Responsibilities of this position include:

  • Acting as the primary point of contact for members and the Interdisciplinary Care Team
  • Monitoring the provision of services as outlined in the Individual Care Plan (ICP) and achievement of desired outcomes
  • Managing assigned case load

Required Skills:

  • Conducts home visits with members
  • Associates: Nursing (Required), Bachelors: Nursing
  • Must be a RN or LPN in the state of Virginia
  • At least 1+ years of clinical experience working directly with individuals who meet Managed Long Term Services
  • Must have 1 year related experience with medically complex members dependent upon life sustaining equipment
