A healthcare company is searching for a person to fill their position for a Remote Self Directed Personal Care Registered Nurse in Winnebago County.

Core Responsibilities of this position include:

• Providing training to participants or care providers as requested and provides educational materials as needed
• Overseeing a My Cares Group of participants
• Documenting assessment as required

Required Skills:

• Must possess a valid driver's license, maintain adequate auto insurance for job-related travel, and ability to travel
• Wisconsin RN license
• Demonstrated computer and software skills required
• 1+ years of home care nursing experience serving individuals with intellectual disabilities, physical disabilities or older adults
• Strong teaching and mentoring skills
• Strong analytical and problem-solving skills
