Job Summary
A healthcare company is searching for a person to fill their position for a Remote Serious Mental Illness Care Coordinator.
Core Responsibilities Include:
- Providing care coordination to members
- Conducting in depth health risk assessment
- Communicating and developing the care plan
Applicants must meet the following qualifications:
- Ability to travel for face-to-face home visits
- GED or High School Diploma
- 3-5 years of clinical experience working directly with MLTSS iindividuals
- Experience in utilization management, quality assurance, home OR
- Facility care, community health, long term care or occupational health
- Valid in state driver's license
