Job Summary

A healthcare company is searching for a person to fill their position for a Remote Shared Services After Hours Care Coordinator in Baton Rouge.

Candidates will be responsible for the following:

• Responding to incoming telephone calls and other inquiries from patient/patients’ families
• Documenting incoming patient telephone calls, response or direction, and the coordination of services
• Making patient care decisions in response to incoming patient telephone calls

Position Requirements Include:

• Valid Driver’s License and reliable transportation and agency required liability insurance
• Current and unencumbered license to practice as a Registered Nurse in all states in the assigned territory
• 3 years of experience as a registered nurse
• 1 year experience in providing direct patient care in the home health/hospice setting
• Current CPR certification
• Able to assess patient status and identify requirements relative to age specific needs
