Job Summary
A non-profit health insurance company specializing in public healthcare programs has an open position for a Remote Social Services Case Manager.
Candidates will be responsible for the following:
- Engaging the member & completing a health & psychosocial assessment, taking into account the cultural & linguistic needs
- Facilitating regularly scheduled inter-disciplinary care team (ICT) meetings to meet the needs of the member
- Engaging with the member to establish an effective, professional relationship via telephonic or electronic communication
Skills and Requirements Include:
- Bachelor’s degree in a health care field or equivalent years of relevant work experience is required
- Licensure as a Social Worker or Counselor is required
- Minimum of 3 years of experience in social work or counseling is required
- Intermediate proficiency level with Microsoft Office, including Outlook, Word and Excel
- Ability to communicate effectively with a diverse group of individuals
