Job Summary

A health insurance company is searching for a person to fill their position for a Remote Social Work Case Manager.

Must be able to:

• Manage behavioral and psychosocial needs that result in improved clinical and financial outcomes
• Ensure effective psychosocial intervention, positively impacting a patient's ability to manage his/her chronic illness
• Utilize available community, government, and/or client resources needed to address participant's limitations

Applicants must meet the following qualifications:

• In the future, you will work from home and up to 50% of the time in the field in the Southeastern part of Nebraska
• Current unrestricted LCSW (or equivalent) license in applicable state(s) required
• Proficient using Microsoft programs
• Requires an MS in Social Work; 3 years of experience, or any combination of education and experience
