Job Summary

A diversified managed health care company is searching for a person to fill their position for a Remote Social Work Field Care Coordinator.

Candidates will be responsible for the following:

• Visiting Medicaid members in their homes and or other settings, including community centers, hospitals or providers' offices
• Providing a complete continuum of quality care through close communication with members via in-person or on-phone interaction
• Supporting members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels

Qualifications for this position include:

• Registered Nurse (RN) with active, unrestricted license in the state of Tennessee
• Three years clinical experience in a hospital, acute care, home health, hospice, direct care or case management
• Five years of experience working in a healthcare environment
• Reliable transportation to travel to member homes or other locations within service delivery area
