Job Summary

A care management company is in need of a Remote Social Worker.

Candidates will be responsible for the following:

• Providing telephone follow-up to ensure members have seen their PCP
• Acting as a liaison and member advocate between the member/family, physician and facilities/agencies
• Coordinating community resources with emphasis on the development of natural support system

Position Requirements Include:

• Requires a Bachelor's Degree in Social Work (BSW), Psychology, Counseling, Rehabilitation
• 2+ years of experience in social work experience
• Licensure/Certification: LCSW or LMSW
