Job Summary

A company is looking for a Social Worker Care Coordinator.

Key Responsibilities:

• Maintain a caseload of patients and provide clinical care management services
• Develop and implement care plans, collaborate with healthcare professionals, and perform patient outreach
• Identify and execute referrals to care and community resources, provide resource management, and act as a patient advocate

Required Qualifications:

• Bachelor's degree in Social Work
• Master's degree or state licensure as required
• 2-3 years of acute care, home health, or case management experience
• Case Management certification, LSW or LCSW preferred
• Experience with database entry, EMR documentation, and basic Excel skills
