Job Summary
A healthcare provider is seeking a Remote Special Needs Program Health Plan Health Manager.
Individual must be able to fulfill the following responsibilities:
- Provide members, providers, and other stakeholders with information concerning benefits and coverage
- Monitor and evaluate effectiveness and outcome of treatment plans
- Utilize supervision by identifying and reporting to supervisor clinical, utilization and outcomes issues
Applicants must meet the following qualifications:
- This position will be predominantly work from home
- Licensed Pennsylvania RN with 6-8 years behavioral health clinical experience
- Five years of experience in clinical, care coordination, and/or case management required
- General knowledge of best practices in behavioral health
- Computer proficiency required
- Knowledge of community resources required
