Job Summary

Would you like to be part of a highly skilled team of home-based Nurse Case Manager (NCM)s providing telephonic case management, focused on supporting individuals in maximizing their health and productivity?

Nurse Case Manager Responsibilities

As a Nurse Case Manager, you will work independently in your home office setting while being part of a supportive nationwide team. Through the application of a unique mix of experience and certification, you will support patients with diagnoses in the fields of occupational-related injuries and illness, emphasize timely facilitation and coordination of diagnosis, and be involved in the acute and chronic phases of cancer treatment, and support. Your broad responsibilities will include developing a case management plan for each injured patient throughout the various stages of recovery while tracking in a proprietary case management system. You will implement integrated medical disability case management services with the goal of preventing, minimizing, or overcoming a chronic disability. Additionally, you will provide medical expertise and serve as the critical communication link between the parties involved in complex medical disability cases. You will ensure timely interventions and facilitate continuous communication while providing patient advocacy through supporting patient choices.

Nurse Case Manager Qualifications

Bachelors in Science Nursing or Associate in Nursing (ASN) or Nursing Diploma.
Unrestricted State Registered Nursing License
3 years of case management experience
3 years related clinical experience in discharge planning, oncology, utilization review/management to determine the necessity of medical services, behavioral health, workers' compensation, occupational health, and/or a comparable field.
Preference is given to Nurses with National Certifications in case management or related field. National Certification may be obtained within 12 months of the date of hire
Attention to detail, timetables, and commitment to completing tasks
Experience with Microsoft Windows and computer savvy
Must be well organized, efficient, and able to work independently and within a team
Responsible for having reliable High-Speed Cable or Fiber Optic Internet service and Internet Router in established home office
Must have Excellent Communication Skills via Phone, E-Mail, Text, Verbal, Documentation Skills, and provide 24 hours follow up to all communication
U.S. Citizenship
Prior to hiring and training able to pass a preliminary credit and background check
