A nonprofit health services organization has an open position for a Remote Stroke Neurology Registered Nurse Care Coordinator in Edina.

Core Responsibilities of this position include:

• Serving as single point of contact for stroke patients and their families
• Coordinating care across the health care continuum for the stroke population

Qualifications Include:

• Able to attend primary company location, as needed
• 3+ years of clinical nursing experience
• Current Minnesota RN-license
• BLS certification or certification within orientation period
• BSN OR an ADN and the completion of a BSN program within 4 years of hire
