Job Summary

A health insurance company has a current position open for a Remote Telephonic Behavioral Health Care Coordinator.

Core Responsibilities of this position include:

• Assess and evaluate members' needs and requirements to achieve and/or maintain optimal wellness state
• Guide members/families toward and facilitate interaction with resources appropriate for the care and well being of members
• Interpret and independently determine the appropriate courses of action

Qualifications Include:

• Master's degree in a behavioral-health field, such as social work, counseling, or related health discipline from an accredited university
• 3-5 years of experience working as a behavioral health professional
• Experience with behavioral change, health promotion, coaching and wellness
• Proficiency with Microsoft Office Programs, e.g. Word, Excel, PowerPoint, Access
• Experience supporting patients telephonically preferred
