Job Summary

A health insurance and wellness company has an open position for a Remote Telephonic Behavioral Health Social Services Support Care Manager 2.

Must be able to:

  • Assist members with identified Social Determinant of Health needs
  • Assess and evaluate member needs
  • Complete required assessments

Position Requirements Include:

  • Master's degree in Social Work from an accredited university
  • Licensure in Master's degree field of study required
  • Must have passed Master, Advanced Generalist, or Clinical ASWB Exam
  • 1 year of managed care experience
  • 3 - 5 years of experience working as a licensed Social Worker
