Job Summary

A health insurance company has a current position open for a Remote Telephonic Care Manager RN.

Individual must be able to fulfill the following responsibilities:

• Manage a member's physical, environmental and psycho-social health issues
• Identify and resolve barriers that hinder effective care
• Monitor patient care through assessments and/or evaluations

Skills and Requirements Include:

• Associates, Bachelor's degree or work equivalent
• 3+ years of varied clinical nursing experience
• Licensed Registered Nurse (RN) in the state of Texas with no disciplinary action
• Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
• Must have accessibility to high speed DSL or Cable modem for a home office
