Job Summary
A health insurance company is seeking a Remote Telephonic Complex Case Manager Registered Nurse.
Must be able to:
- Make inbound and outbound calls to assess members' current health status
- Identify gaps or barriers in treatment plans
- Providepatient education to assist with self-management
Skills and Requirements Include:
- Must live within a commutable distance to the Kingston, NY or Albany, NY office
- Current, unrestricted RN license in the State of New York
- 3+ years of experience in a hospital setting, acute care, direct care, or as a telephonic Case Manager
- Multiple state licensures or the ability to obtain multiple state nursing licenses
- Strong computer proficiency
- Access to high speed internet at home
