Job Summary
A healthcare company has an open position for a Remote Telephonic Health Care Navigator.
Core Responsibilities Include:
- Completing telephonic outreach to members utilizing auto dialer and manual dialing
- Educating member on gaps in care and assists with scheduling provider appointments
- Assisting members with social determinants of health and links to community resources
Qualifications for this position include:
- High School Diploma / GED (or higher)
- 1+ years of call center and / or telephonic customer service experience
- 1+ years of professional experience in an office setting using the telephone and computer
- 1+ years of experience in analyzing and solving customer problems
- 1+ years of Healthcare/insurance/Social work/community outreach/ or advocacy experience
- Work experience using Microsoft Word, Excel, and Outlook
