Job Summary
A healthcare company is searching for a person to fill their position for a Remote Telephonic HIV Case Manager Registered Nurse.
Must be able to:
- Make outbound calls and receive inbound calls to assess members' current health status
- Identify gaps or barriers in treatment plans
- Provide patient education to assist with self-management
Must meet the following requirements for consideration:
- Current, unrestricted, active RN license in the State of New Jersey
- 3+ years of experience in a hospital, acute care or direct care setting
- Must be able to type and have the ability to navigate a Windows based environment
