A healthcare company is filling a position for a Remote Telephonic Nurse Care Manager in Westborough.

Individual must be able to fulfill the following responsibilities:

  • Act as an advocate for the patient in all activities
  • Engage with families/responsible parties to collaborate on plan of care
  • Act as liaison between providers, nursing facilities, hospitals and program staff

Qualifications Include:

  • Current RN License
  • 3+ years of clinical practice in a hospital, clinic, home care, or nursing home setting highly desired
  • Ability to identify and report any quality-of-care issues and maintain HIPAA compliance
