Job Summary
A healthcare company has an open position for a Remote Telephonic Nurse Care Manger I or II.
Candidates will be responsible for the following:
- Ensuring member access to services appropriate to their health needs
- Conducting assessments to identify individual needs and a specific care management plan
- Coordinating internal and external resources to meet identified needs
Position Requirements Include:
- BA/BS in a health related field or
- 3 years of clinical experience or
- Any combination of education and experience
- Current, unrestricted RN license in applicable state(s) required
- Minimum 2 years "telephonic" Case Management experience with a Managed Care Company
